Once booked, you’ll receive a welcome email with next steps — including the My~LifeStory Questionnaire and photo upload instructions.

You’ll receive a link to complete the My~LifeStory Questionnaire — a simple, heartfelt form that helps us shape a tribute using real memories, milestones, and personal reflections.
Whether filled out directly by the person themselves, or with the help of a loved one, these details allow us to create something truly personal and meaningful.

Once your questionnaire is complete, it’s time to submit your photos.
If You Have Digital Photos:
Use the secure upload link provided in your confirmation email.
You’ll also receive a sharable upload link to send to friends and family so they can easily contribute their own images as well.Prefer to email files or use a different method? We’re flexible — just let us know.
If You Selected the Photo Scanning Service:
We’ll reach out to schedule your pickup or drop-off and scan your printed photos for you — no uploads required. -
Once we receive your materials, we’ll begin crafting a meaningful, personalized video using your photos, music, and any text or quotes you’ve provided.
